This accident report card provides details of the fatal crash of a Blenheim IV of 17 Operational Training Unit on 12 June 1940. The pilot, V.A. Bain parachuted to safety. Sadly W.H. Wheeler and Sidney Boulton, the other two crew members weren't wearing parachutes and were killed in the crash. The aircraft was reported to have broken up in flight and spun in from 20,000ft.
